Carolyn Perrotti Lake

Dr. Lake has 10 years of experience in clinical practice. She holds a Bachelor of Science Degree with a Minor in Biology from Gettysburg College, a Master's of Science in Nursing, Family Nurse Practitioner Specialty, and Diabetes Care Concentration from Yale School of Nursing, and Doctor of Nursing Practice, Psychiatric Mental Health Nurse Practitioner Specialty, and Master's of Public Health from the University of San Francisco. She is board certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners and as a Psychiatric Mental Health Nurse Practitioner through the American Nurse's Credentialing Center.

Dr. Lake has experience working in primary care, urgent care, pediatrics, cardiology, and occupational health. Dr. Lake served as an officer in the Nurse Corps of the United States Air Force and currently works in clinical practice serving our nation's Veterans.
