Eligibility Rules

Effective January 1, 2014, new hires will be eligible for medical, dental, vision and flexible spending accounts on the first of the month following the date of hire.

Anthem Blue Cross changes to Compound Pharmacy

Anthem Blue Cross will make a change to how it pays for certain compound drugs. This does not impact all compound drugs, but those drugs being produced using non-FDA approved ingredients will no longer be covered. Compound drugs whose primary ingredient is FDA-approved and the end-use is consistent with the on-label indication will continue to be covered. There are usually several alternatives to these types of medications that are covered and are FDA approved. Pharmacies and physicians understand that there are alternatives to using off-label compound products.

Spousal Surcharge

Effective January 2013, we implemented a $75 per month surcharge on medical plan premiums for faculty and staff whose spouse, Registered Domestic Partner (RDP) Grandfathered Legally Domiciled Adult (GF LDA) dependent have medical coverage available through his/her employer but who are nevertheless are included as a dependent of the employee. All faculty and staff with an enrolled spouse, RDP, or GF LDA will pay the surcharge unless a declaration is returned Human Resources, Benefits team, by November 15th, 2013.

An employee will not be responsible for the healthcare surcharge for a spouse/RDP if the employee's spouse's/RDP's health plan through his/her employer does not meet the ACA's minimum essential coverage requirements, or if the spouse's/RDP's plan does not meet the ACA's minimum value requirements of 60%. Either of these facts must be confirmed by letter from the spouse's/RDP's employer to USF Benefits staff together with the Summary Plan Description from that plan. Annual re-certification is required.

Spousal Surcharge Waiver Declaration

Annual re-certification of the spousal surcharge waiver is required. A new spousal surcharge waiver form must be received by November 15th, 2013 for the 2014 plan year.
