Fall Wellness Incentive

Did you participate in an on-campus biometric screening in September, October or November?

If you did, you completed Step 1 of the Fall Wellness Incentive. Step 2 is to take your Personal Health Profile (PHP). The link to get you started is http://www.usfca.edu/hr/wellness/php.

When you complete this step, you will receive a 1 year subscription to Prevention magazine.

Engage with a health coach to review your PHP and screening results and you will have completed Step 3 and you are eligible for a $50 credit at Koret, the USF Bookstore or Bon Appetit.

Clipper Direct will soon replace Commuter Checks

Clipper Card

Effective January 1, 2012, Clipper Direct will replace Commuter Checks in the administration of the commuter subsidy benefit.

You will receive an email to enroll in Clipper Direct on November 16th. On November 17th and 18th, Clipper Direct will be on campus to help you activate your account. Come to the Human Resources office (LM339) between 9am and 4:30pm to speak with representatives from Clipper Direct.

If you enroll in Clipper Direct and contribute at least $5 per month via a pre-tax payroll deduction, and you do not have a University parking permit, the University will contribute $65 per month to your Clipper Direct account, if you are a benefits eligible employee. Clipper card can be used for specific transit passes, for a cash value to be used on a per-ride basis, or both.

If you already have a Clipper Card you can link your existing card to this new account. If you do not already have a Clipper Card, you will receive one with your first month’s order.

Auto Enroll is Coming...

You may have received information about Auto Enrollment, a new feature of the 403(b) plan with TIAA-CREF.  If you do not currently contribute or contribute at less than 3%, this affects you.

If you want to opt out of Auto Enrollment, please fill out the 403(b) Opt Out Form available at www.usfca.edu/hr/benefits by 5:00 p.m. on Friday, December 16, 2011.

In addition, the IRS limits for 2012 for employee contributions to a 403(b) Plan will increase from $16,500 to $17,000 for employees under 50. For those 50 and over, the additional $5,500 catch-up provision remains in effect.

If you want to contribute the maximum in 2012 and do not currently, or you want to change your current amount, please complete an Authorization for Salary Reduction form available at www.usfca.edu/hr/forms and submit it to Human Resources by January 4, 2012.

Making Changes to Your 2011 Contributions to the 403(b) Plan

With only a few pay periods remaining for the 2011 calendar year, if you would like to change your contribution to the 403(b) plan for your December payroll check(s), please submit an Authorization for Salary Reduction form found at www.usfca.edu/hr/forms to Human Resources by December 2, 2011 at 5:00 pm.
