Procedures

1. Applicant submits two copies of the application to the IRB office, including:
   1. Signatures
   2. Copies of surveys, tests, questionnaires and other instruments used to gather information
   3. Permission letters from organizations where subjects will be obtained
   4. Informed consent(s) or cover letter(s) in IRB format
2. Notification of complete/incomplete application will be sent via e-mail to applicant
3. Primary reviewer will notify applicant of any modifications needed
4. Applicant will be notified of results of review by IRB office within 2-3 weeks after application is received.

Please include all "elements of consent" (p. 32 of IRB Manual) and Appendices A-J as templates for forms.
