LOST
USF ID CARD DEACTIVATION |
|
If
you lose or misplace your USF ID Card, call the One
Card Office immediately to have it deactivated. You
may be responsible for transactions made with your USF
ID if lost depending on when you reported it lost. Calling
to report your ID Card missing immediately after losing
it will minimize the potential for fraudulent use and
your liability.
You
can all any day of the week, at anytime to deactivate
your lost USF ID Card.

REPLACING
YOUR USF ID CARD |
|
- Once
a USF ID Card is replaced, it cannot
be reactivated.
- If
you deactivate (but do have not
yet replaced) a lost USF ID Card and then find the same card, bring it to the One Card Office
to have it reactivated. There is no fee to have this
card reactivated.
- If
you can not find your lost USF ID Card, there is a
$10 replacement fee, regardless if it was lost, stolen
or misplaced.
- If
you lose your current USF ID Card but find an older
one, it cannot be reactivated for use. However,
you may exchange the older card for a $5 credit towards
the $10 replacement fee.
- Damaged
cards due to normal wear and tear are replaced at
no charge provided that the damaged card is exchanged
at time of replacement. USF ID Cards with hole punches,
washed in laundry, commingled with other items, etc.
are charged the replacement fee. USF ID Cards should
be kept protected and separate from keys and other
cards.

GENERAL INFORMATION |

The Access, Entitlements, and Debit Accounts are not
physically on the card. Rather, they are accessed by
swiping the USF ID Card which will verify and then debit
the Access, Entitlement, or Funds that are available to
the cardholder. If the card is lost or misplaced, we strongly
recommend reporting it lost to
freeze the account and prevent fraudulent use. If a
replacement card is needed, all the funds (and other
card based services) are automatically transferred to
the replacement card. A PIN is not required to make a
purchase, but some locations such as the USF Bookstore
may require a signature. A PIN is required to access
individual Res Hall doors. |
