Lost Cards & Replacement Cards

DEACTIVATING MY USF ONE CARD
|
- If
you lose or misplace your One Card, call the One
Card Office immediately to have it deactivated
- You
may be responsible for transactions made with your
USF One Card while it is lost. Calling
to report it as missing will minimize both the potential for fraudulent use, as well as your liability

REPLACING
MY USF ONE CARD IF I ATTEND/WORK AT THE USF MAIN CAMPUS |

- Once
a USF One Card is replaced, it cannot
be reactivated
- If
you deactivate (but have not
yet replaced) a lost USF One Card and then find it, simply bring the found card to the One Card and Campus Security Systems Office, Lone Mountain 130,
to have it reactivated. There is no fee to have a current
card reactivated
- If
you can not find your USF One Card, there is a $15 replacement fee, regardless of whether it was lost, stolen,
or misplaced
- If
you lose your current USF One Card but find an older
one, it cannot be reactivated for use. However,
you may exchange the older card for a $10 credit towards
the $15 replacement fee
- Damaged
USF One Cards due to normal wear and tear are replaced at
no charge, provided that the damaged card is exchanged
at time of replacement. Please note that for USF One Cards that have been hole punched,
washed in the laundry, or commingled with other items, you will be charged the standard replacement fee
- As a reminder, USF One Cards should
be kept protected and separate from keys and other
cards

- USF One Cards may be replaced by mailing a check, made out to University of San francisco, in the amount of $15 (replacement fee), to:
University of San Francisco
One Card and Campus Security Systems Office
Lone Mountain 130
2130 Fulton Street
San Francisco, CA 94117
- Once payment has been received, your USF One Card will be mailed to your regional campus for you to pick up

GENERAL INFORMATION |
|
- Access, entitlements, and debit accounts are not
on your USF One Card itself. Rather, they are accessed when you
swipe your USF One Card, which verifies and debits
the access, entitlement, or funds that are available to you
- Should you need to replace your USF One Card, all funds (and other
card-based services) are automatically transferred to
the replacement card
- A PIN is not required for purchases made with your USF One Card, but some locations, such as the USF Bookstore,
may require your signature
- A PIN is required to access
individual resident hall doors
