Required Immunizations

The University of San Francisco requires evidence of MMR  (Measles, Mumps, Rubella)immunization for students in the following categories:

  • Domestic undergraduate students born on or after January 1, 1957 (excluding Working Adult Professional Studies/Legacy CPS)
  • Domestic graduate students born on or after January 1, 1957 living on campus
  • All international students
    • International Students are also required to show proof of Tuberculosis  PPD (Mantoux) test administered in the last 12 months. Students with positive PPD are required to submit a chest x-ray administered within the last 12 months. 

Students who plan to live in University-operated housing will not be admitted to their residence hall until their Immunization Form/records are submitted.

Non residential students with incomplete immunization records will have a registration hold placed on their second semester registration.

Nursing students are required to get the blood titters or show proof of immunity for MMR as required by the Nursing Department.  

Evidence of immunization must be submitted in writing from a licensed medical professional using the USF Immunization Form. The Immunization Form and records may submitted by mail, fax, or email:

All students are strongly encouraged to keep a copy of their immunization records for future reference.

The USF Student Health Clinic is able to provide all required immunizations and tests to those students needing them. For more information about the immunization requirement, please contact the USF Clinic at 415-750-4980.

Exceptions for immunization: Permitted when conflicts with personal religious belief or documentation by a physician that immunization should not be given due to medical condition.
