Spring/Summer open enrollment ends on February 3, 2012, at 5:00 p.m. Graduate students who are required or eligible to purchase the USF-sponsored health insurance plan should follow these steps to enroll:
- Go to the Aetna Member Navigator Website.
- In the left navigation menu, click on "Medical Plan: Enroll."
- Click on the submenu "Graduate Students."
- Click on "Enroll: Graduate Students."
- Follow the prompts to enter all the necessary information to enroll.
- After entering all the necessary information, pay using a Visa or Mastercard.
Important Dates:
- Summer 2011 coverage will be available for purchase on May 19, 2011.
- Fall 2011 coverage will be available for purchase on July 1, 2011.
Waive
The waiver period for Spring/Summer 2012 ends on February 3, 2012, at 5:00 p.m. Students who already have a health insurance plan comparable to or better than the USF-sponsored health insurance may petition to waive the health insurance requirement.
Deadline
Students who wish to waive the insurance requirement must do so by the deadline. The deadline for each term is 7 days before the University Census Date of that semester.
Upcoming deadlines:
- Spring 2012: February 3, 2012
- Fall 2012: August 31, 2012
- Spring 2013: February 1, 2013
U.S. Citizens and Permanent Residents
Criteria for waiving the health insurance requirement:
- Health plan must be provided by a company licensed to do business in the United States, with a U.S. claims payment office and a U.S. phone number.
- Health plan must offer a maximum benefit of at least $100,000 in coverage per benefit year.
- Health plan must cover inpatient and outpatient medical care within the San Francisco Bay Area. Note: Emergency-only coverage does not satisfy this requirement.
- Health plan must cover inpatient and outpatient mental health care within the San Francisco Bay Area. Note: Emergency-only coverage does not satisfy this requirement.
- Health plan must be active and provide health insurance coverage throughout the academic year.
Instructions on how to submit the waiver:
- Go to the Aetna Navigator Member Website.
- In the left navigation menu, click on "Medical Plan: Waive."
- Scroll down and click on "Waive: Student Health Insurance Plan."
- Follow the prompts to enter all the necessary information for your waiver.

- Reliance General Insurance
- Travel and Accident plans by United Health Care
These health insurance plans are NOT accepted because they have many limitations and exclusions for medical services.
International Students
To waive the insurance requirement, your insurance plan must be comparable or better than the USF Sponsored Health insurance plan.
Criteria for waiving the health insurance requirement. Please be advised these are general criteria and all health plans will be reviewed by the University insurance broker to ensure the plan meets USF requirment.
- Policy must be in English.
- Policy must be underwritten by a U.S.-domiciled (U.S.-based) insurance company.
- Policy must include accident and full medical coverage (include illness/preventive care, prescription, no limit on hospital stay/emergency room except for the co-insurance and deductible).
- Health plan must offer a maximum benefit of at least $100,000 in coverage per benefit year.
- Deductible no greater than $200 per accident or illness.
- Medical evacuation minimum of $10,000 in coverage.
- Repatriation minimum of $7,500 in coverage.
- Pre-existing conditions must be covered after a reasonable waiting period (not to exceed six months)
- Includes provision for co-payment that does not exceed 10%.
- Coverage cannot exclude benefits for perils inherent to the activities of the program in which the visitor participates.
Instructions on how to submit the waiver:
- Download the Health Insurance Waiver Form for International Students (PDF).
- Fill out the form completely, including full name, student ID, email address, and insurance information.
- Attach proof of your insurance coverage. Proof must be in the form of a statement of coverage in English.
Please note: Waivers that do not include sufficient documentation will not be approved.
- Submit the form to Health Promotion Services in University Center, fifth floor.
