Department or Group Course Request Form

USFEMR Service provides Heartsaver® CPR AED and Heartsaver® First Aid courses for USF campus groups and departments. In order to request a group class there must be at least 10 participants and the class must be scheduled and approved 14 days in advance. The USFEMRs will staff and provide for accommodations for the group when space and staffing is available.

The fees per student are as follows:
$65/student for 15 students or less
$60/student for 16-20 students
$55/student for 21-25 students
$50/student for 26 or more students

Interested departments are ask to submit a detailed request form more than a 14 days in advance to ensure availability as well as a student roster with the students' name, mailing address, e-mail and phone number. Payment must be received 1 week before the course date. Because of limited staffing, an additional charge of $150.00 is added for time scheduled less than a week in advance and for cancellations less than 48 hours prior to a scheduled class.
