Requirements
USF has different requirements for domestic undergraduate, domestic graduate, and international students. Read the table below to find out more information on each.

Deadlines
The deadline to submit proof of required immunizations depends on your term of admission:
- Fall 2012: September 1, 2012 at 5 PM.
- Spring 2013: February 1, 2013 at 5 PM.
Failure to submit proof of immunizations by the appropriate deadline will result in a $100 fine and may also result in a registration hold on your student account.
Note: The deadline for IEP students is different. IEP students must submit proof of immunization by the second week of enrollment. Failure to meet this deadline will result in a $100 fine.

How to Submit Proof
Download the USF Immunization Form.
Proof of immunization must be obtained from a licensed medical professional using the USF immunization form, and submitted to Health Promotion Services by mail, fax, or email.

If you are unable to obtain proof of immunization before you arrive at USF, you may visit the USF Student Health Clinic to receive all required immunizations.

Directions
To access the USF Student Health Clinic: Enter the main entrance of St. Mary's Medical Center located on Stanyan Street. Make a left towards the elevators and take the elevators to the 2nd floor. Turn left when you get off the elevators.

Exceptions
Exceptions to the immunization requirement may be granted for the following reasons:
- Conflict with personal religious belief. Student must sign an affidavit affirming that the use of immunization conflicts with a strong, personal religious belief.
- Conflict with medical condition. Student must provide documentation from a licensed medical professional that immunization should not be given due to a medical condition.
