1. Download the forms from the links below by right clicking on them (mac: command click), and selecting the option to save to your hard drive
2. Open the forms in Adobe Reader
3. Fill out the required information and click file --> save while in Adobe Reader
4. Email the saved forms, along with any scanned receipts to usflawtreasurer@gmail.com
