Facility
User Insurance
When an outside group wants to use a university facility for a
meeting or event, the group needs to sign an agreement promising that they will
be financially responsible for any loss arising from their use of the facility.
The group must also provide evidence of insurance naming the University of San
Francisco as additional insured. Please contact the Lone Mountain Conference
Center at x6166 for more information. A copy of the requested certificate of
insurance should be forwarded to Anthony Retort, Office of Accounting & Business Services.
Sports
Camp/Kids Camp/Youth University/Upward Bound
For camps or programs that our sponsored by the University of
San Francisco, insurance must be purchased for the participants. Please contact
CPSJ Insurance Group at (408) 554-2755 for insurance information.
Contract Review
All contracts need to be reviewed by both the Office of Legal
Counsel and the Office of Accounting & Business Services, prior to signing. Please
submit a copy of the contract to both offices for review. For this review process,
please allow at least ten business days.
Certificate
of Insurance
When the university enters into a contract/agreement that
requires the university to provide a certificate of insurance, please forward
the request and a copy of the contract/agreement to Anthony Retort, Risk
Analyst at x5899, Office of Accounting & Business Services.
The following information is needed to comply with the request:

A certificate will be requested from the university's insurance
broker and forwarded to the company via fax and mail. Please allow at least
five business days for processing.
When other parties enter into contracts with the university and
the university has requested proof of insurance, please forward a copy of these
certificates of insurance to the Office of Accounting & Business Services, Attention:
Anthony Retort, Risk Analyst.
Property
Insurance
The University carries blanket property insurance on University
property to protect against damage/loss of property. If an incident occurs
involving university property, please contact the Public Safety department to
file a report and Anthony Retort, x5899 in the Office of Accounting & Business Services. The University will fund the repair or replacement cost of University
property, less a $250 deductible - borne by the department responsible for the
property.
The University reserves the right to seek reimbursement from the
authorized individual/department if an investigation shows misuse of said
property or negligence was involved that resulted in the incident.
Driver's
Record
University
of San Francisco Pull-Notice Program
Authorized drivers shall grant the University permission to
obtain a copy of their driving record.
Students and volunteers shall obtain a copy of their driving
record at the Department of Motor Vehicles prior to being assigned driver
duties. The driver's record must be in compliance with the provisions stated
herein. A copy of the record should be forwarded to Anthony Retort, Risk
Analyst at x5899,Office of Accounting & Business Services, for adding to the
University of San Francisco Pull Notice Program.
The following guidelines shall be used when designating
authorized drivers. Any violation of these guidelines will be cause for review
of one's driving record and/or revocation of University driving privileges:
- Driver
must possess a valid California driver's license and be familiar with
California motor vehicle laws.
- Driver
must not have more than three moving violation points within the last three
years.
- Drivers
must not have any conviction for driving under the influence (DUI) of alcohol
or drugs within the last three years.
- Drivers
must read and understand the provisions of the Driver's Handbook. For a copy of
the Driver's Handbook, please contact Anthony Retort, Risk Analyst at x5899, Office of Accounting & Business Services.
Auto
Insurance/Deductible
The University carries blanket liability insurance on University
vehicles to protect against bodily injury or damage to property of others. The
University funds the repair or replacement cost of University vehicles, less a
$250 deductible - borne by the department responsible for the vehicle. A USF
insurance information card should be stored in the glove compartment of
University vehicles.
The University reserves the right to seek reimbursement from the
authorized driver if an investigation shows misuse of the vehicle or negligence
that results in an accident.
Private vehicles used on University business must be insured as
required by California state law.
What to Do
In Case of Accident:
- STOP AT ONCE. Check for personal injuries and send for an
ambulance, if needed. Do not leave the scene, but ask for the assistance of a
bystander.
- DO NOT ADMIT LIABILITY. Make no statements regarding fault,
liability or payment of bills.
- DO NOT ARGUE OR DISCUSS THE INCIDENT. Speak only with University officials, police or your insurance representative.
- SECURE THE ASSISTANCE of a police officer whenever possible and
record their name and badge number.
- RECORD the names and addresses of all witnesses and occupants of
involved vehicles.
- AT THE SCENE COMPLETE THE ACCIDENT REPORT FORM, found in the

Auto
Rentals
Rental vehicles shall be arranged pursuant to University
guidelines.
At the time of rental, the option of purchasing a collision
policy through the rental agency is offered. The University has a non-owned
auto policy that covers this type of exposure. Therefore, authorized drivers of
commercially rented cars shall not purchase a collision damage waiver from the
agency, when renting in the United States. If the driver desires coverage for
personal effects, the driver may purchase this insurance through the rental
agency at his or her own expense.
Rentals in foreign countries are discouraged; however, if it is
necessary to rent an auto in a foreign country, to ensure compliance with the
laws of the country, the renter shall purchase all insurance coverage
available. Students shall not rent or drive vehicles in a foreign country in
conjunction with University activities.
Adding or
Deleting Vehicles
When a department purchases a vehicle, please provide the year,
make, model, Vehicle Identification Number, and License Plate number to Anthony
Retort, x5899, Office of Accounting & Business Services. A vehicle insurance card will
be issued for that vehicle and forwarded to you along with an Accident Report
Kit brochure.
