One letter of recommendation and recommendation form must be sent directly from the person writing the letter or with your application in a sealed envelope. Please make sure your name and date of birth are on the letter.

This important form is for freshman applicants applying under Regular Action only and must be sent as soon as fall semester grades are available if they are not included on your official transcript.
