Other Admission Requirements

Health Insurance Requirement

The University requires the following groups of students to show proof of current health insurance coverage:

All students living in University-operated housing are required to provide proof of insurance coverage under a United States insurance plan as part of their housing contract. Evidence of coverage should be provided to the Office of Residence Life, and must be in writing, which normally is considered a copy of an insurance identification card or a policy statement (including name of insurance provider, policy number, and company phone number). Coverage must be for the entire academic year. Students who do not have adequate coverage may purchase the USF-Sponsored Student Injury and Sickness Insurance Plan.

Domestic Students

Evidence of coverage must appear in writing. Normally an insurance identification card or a policy statement is considered acceptable for domestic students. Coverage must be for the entire academic year. Students who do not have adequate coverage may purchase the USF-Sponsored Student Injury and Sickness Insurance Plan. Information on the Student Health Insurance Plan and how to furnish "proof of health insurance" is mailed to new and continuing students each June.

International Students

International students on F-1 and J-1 visas AND registering for classes in the Fall and Spring semesters are automatically enrolled in and covered by the University-sponsored Student Injury and Sickness Insurance Plan. The cost for this coverage is included on the student's account in both the Fall and Spring semesters. Students enrolled for classes in the Fall will have coverage through mid-January, and students enrolled in the Spring will have insurance benefits extended through mid-August.

International students who are able to prove that they have comparable coverage under a United States health insurance plan may waive this requirement. Proof must be in the form of a statement of coverage and an insurance identification card for the applicable period of enrollment and submitted to the Student Health Insurance Office (University Center 200) within five class days from the start of each semester. If coverage is deemed acceptable, the student's financial account will be credited accordingly. Students should not purchase insurance from their home countries because it will not meet the University's requirement.

International students who plan to enroll during Summer Session but were not enrolled in the preceding Spring semester are NOT automatically enrolled in the University injury and accident plan for the Summer. These students, therefore, must show proof of current health insurance. Proof of coverage must be written in English and include benefits comparable to the plan offered by the University. If proof is not acceptable (e.g., coverage cannot be determined or benefits cannot be verified), the student must purchase the University plan for the Summer term.

University Immunization Policy

The University of San Francisco requires evidence of immunization for students who fall into any of the following categories:

If proof of immunizations for measles, mumps, and/or rubella is not available, a blood Titer showing immunity will be accepted.

International Students Only

International students are also required to show proof of a Tuberculosis PPD (Mantoux) test administered in the last 12 months. Students with positive PPD are required to submit tests of a chest x-ray administered within the last 12 months.

Exceptions

Exceptions are permitted in cases of conflict with personal religious belief or documentation by a physician that immunization should not be given due to medical condition.

Recommended Vaccinations:

Evidence of immunization must be submitted in writing from a licensed medical professional. All students accepted to USF should receive an immunization form in their admissions packet. (Additional copies of the Immunization Form can be downloaded at www.usfca.edu/hps/immunize/immunizationform.pdf.) The Immunization Form must be submitted prior to registering for classes. REGISTRATION HOLDS WILL BE PLACED IF IMMUNIZATION REQUIREMENTS ARE INCOMPLETE.
