Administrators may purchase optional term life insurance which
provides coverage in addition to the basic group term life insurance
policy. Employees may choose coverage in an amount equal to 1x, 2x, or
3x the annual base salary rounded to the next $1,000. Administrators
may also select coverage for spouses and dependent children. These
age-based premiums are paid by the administrators through payroll
deduction.
USF provides group term life insurance protection in the amount equal
to an administrator's annual base salary rounded up to the next
$1,000. Accidental Death and Dismemberment Insurance is also provided in
this amount. Under Accidental Death and Dismemberment coverage, the
insurance company will pay a benefit if an administrator sustains
certain bodily injuries while insured under the policy. The injuries
must result directly from an accident independent of all other causes.
