Health Insurance Claim Form Off-Campus Student
Current Graduate, Working Adult Professional Studies/Legacy CPS, Regional and Visiting students registered for at least 6 units are eligible to purchase the University-sponsored annual or semester health insurance plan. Students seeking summer only coverage must be enrolled in at least three summer unit.
Note: You must be a student at USF enrolled in at least 6 units in order to purchase health insurance.
On-Campus Students
Graduate, Working Adult Professional Studies/Legacy CPS, Regional and Visiting students who reside in the University-operated housing are not automatically enrolled in and not billed for the University-sponsored health plan. These students must purchase the coverage or apply for a waiver (see Office of Residence Life, UC 5th floor)) in order to comply with the USF health insurance policy.
